The Diagnosis and Oncological Treatment Card includes personal data of the patient

The Diagnosis and Oncological Treatment Card includes personal data of the patient, i.e.:

- name (names) and surname,

- date of birth,

- sex, address,

- Social Security number - PESEL (in case of a newborn – the Social Security number of a parent or legal guardian), and in case of people who do not have a  social security number - the type and number of an identity document,

- In the case of a minor patient, who is completely incapacitated or incapable of informed consent - surname and name(s) of the legal representative or the actual guardian and the address of his/her place of residence.

Apart from the above, the card includes, among others, the designation of the doctor who provided the patient with healthcare services, the date of issuing the card; information on the symptoms and the oncological diagnostics, the diagnosis itself, the cancer treatment plan, date of completion of the cancer treatment
